An ER patient's presentation changed after the initial triage assessment. They looked more unwell and described a change in symptoms, so I reassessed within my role and promptly alerted the charge nurse and the responsible clinical team through the department's process.

I explained what was different from the initial information and what I was concerned about now. I made sure the change was communicated to the person coordinating the patient's next steps and documented the updated observations as required. The patient received a further evaluation. I did not treat the earlier triage category as a permanent description or independently assign a new category outside my role.
