One example that stands out was a patient whose initial presentation seemed lower acuity, but something about the reassessment did not fit the original category anymore. The patient looked more ill, the symptoms were evolving, and the overall picture felt less stable than the first look suggested. I reassessed, escalated the concern, and made sure the patient was moved back into a more urgent workflow instead of simply waiting in the original track.

What mattered was being willing to update the picture quickly instead of staying loyal to the first label. In emergency nursing, the first triage decision is important, but it is not sacred. A strong ER nurse keeps asking whether the patient's current reality still matches the track they are in.
