I would focus on the gap I was seeing, whether that was mobility, support at home, medication understanding, follow-up needs, or a mismatch between the patient's current condition and the discharge plan. Then I would bring that concern to the appropriate members of the team, such as the provider, charge nurse, or case management, depending on the structure of the facility.
I think this is an important advocacy area for nurses because we often see practical barriers that do not always show up clearly in the formal plan. Travel status does not change that responsibility. Even if I am temporary on the unit, I still need to speak up if I believe the patient is being set up for an unsafe transition. The key is to do it with clear observations and a collaborative tone.
