Instruction: Explain how you would assess the situation, prioritize, communicate, and escalate in a real clinical setting.
Context: Assesses whether the candidate can handle a realistic general hospital travel nurse hiring scenario with safe judgment, clear communication, and appropriate escalation.
If I noticed a change that did not line up with the handoff report, I would trust the assessment in front of me and respond accordingly. I would reassess the patient, gather the key data I need, and escalate promptly using a clear communication structure such as SBAR if the situation warranted it. I would also update the charge nurse so the team was aligned.
I would not spend time blaming the previous nurse in that moment. The immediate priority is the patient. After the situation is stabilized, I may clarify the handoff gap if it is important for learning or future safety. Travel nurses step into handoff systems that vary from place to place, so being able to detect changes independently and act quickly is critical. Good handoff matters, but the bedside assessment still has to drive the response.
"If it was not mentioned in report, I would watch it for a while before making a big deal out of it."
