How would you record a dentist’s instructions accurately in a training chart?

This is a fictional software exercise with no real patient data. You have your own training login. The dentist dictates: “Patient reports discomfort with cold drinks on the lower left. Record that as the patient's report. I will review the previous images with them at a follow-up visit. No treatment was performed today.”

The four fields are Patient report, Dentist's plan, Treatment completed today and Next appointment. The next visit has not been booked. In this exercise, trainee entries remain drafts until the dentist reviews them.
